1. How do I get a quote for my prospect?
Baptist Health Services Group will submit the required group information to our contracted insurance carriers for quote requests, and we follow up with the carriers until they generate quotes. We then prepare a presentation binder that includes the quotes we receive as well as specific marketing information about services offered through the BHSG network.

BHSG sales representatives are available to assist you in presenting the information to your client, and we can discuss the BHSG network during employee open enrollment meetings. Each broker who uses BHSG services is assigned to an in-house sales representative who is committed to timely follow up and excellent service to both you and your clients at no cost. 2. What information do you need from me to get a quote?

For groups with two-99 employees:
  • Group name and address.
  • Industry type.
  • Carrier history.
  • Effective date.
  • Employer contribution for employees and dependents.
  • Current benefit plan.
  • Requested benefit plan.
  • Current and renewal rates.
  • Census (including sex, birthdate/age and coverage type –single, spouse, family –for all employees).
  • Information on major medical conditions will make getting the most accurate quote easier.
For groups with 100 or more employees:
  • Type of industry (include SIC if available)
  • Complete census, including date of birth, sex, type of coverage (employee, spouse, number of children) and life insurance amount.
  • Name of present insurance carrier and funding arrangement (i.e. fully insured, self-insured or minimum premium).
  • Three years of premiums and claims experience (preferably the last 12 months broken down monthly). Include employee and dependent rates both for present and renewal.
  • Number of covered employees and dependents for the last 12 months (broken down monthly).
  • Particulars on any individual claims in excess of $10,000, including amount, diagnosis and prognosis.
  • Identify on census if retirees are covered.
  • Details on any covered disabled people.
  • Indicate any union affiliations with this plan.
  • Copy of the current benefit booklet.
  • Percentage of employee and dependent cost paid by the employer.
  • Number of employees who have elected to continue coverage under the Consolidated Omnibus Budget Reconciliation Act (COBRA).
  • Insurance companies for the past five years and how long the group has been with each carrier.

3. What about self-insured prospects?
Baptist Health Services Group can also assist you in obtaining quotes. The following information is needed to quote on groups wanting a self-insured contract:
  • Type of industry (include SIC if available)
  • Complete census, including date of birth, sex, type of coverage (employee, spouse, number of children) and life insurance amount

4. What size group may I submit?
The Baptist Health Services Group sales team will help you market any group with more than two employees. Carriers release quotes based on group size. Therefore, the quote selection you receive will be based on the criteria of the carriers to whom the information is submitted.
